Procedures for Requesting Disclosure of Personal Information

1. In the event a request is made for notification of the purpose of disclosure or use

In the event you are going to make the following request (hereinafter referred to as "request for disclosure") to us pursuant to Article 32 of the Personal Information Protection Law, Article 24 (Procedures for Responding to a Demand) of the Guidelines, and Article 10 (Request for Disclosure, etc.) of our Privacy Policy, please refer to the following.


  1. (1)Notification of private-information usage purposes
    Paragraph 2 of Article 27 of the Personal Information Protection Law, and Paragraph 2 of Article 19 of the Guidelines

  2. (2)Disclosure of Personal Information Article 20,
    Paragraph 1 of Article 28 of the Personal Information Protection Law, and Paragraph 1 of Article 20 of the Guidelines

  3. (3)Correction, Addition, or Deletion of Personal Information
    Paragraph 1 of Article 29 of the Personal Information Protection Law, and Paragraph 1 of Article 21 of the Guidelines

  4. (4)Suspension of use, deletion, or suspension of provision of personal information to a third party
    Paragraphs 1 and 3 of Article 30 of the Personal Information Protection Law, and Paragraphs 1 and 3 of Article 22 of the Guidelines

3.Necessary Documents

When requesting disclosure, customer is requested to fill out all of the items specified in the following request form (A), enclose the document for identification (B) and the fee (C), and send it by mail to the above counter.


(A)Invoice

  1. (1)Intended Use Notice: Intended Use Notice Invoice                Download

  3. (3)Correction, deletion, etc.: Invoice for correction of personal information   Download

※ However, the maximum number of bills per invoice is up to 3.


(B)Documents for Identity Confirmation

Customer is requested to attach both (1) and (2) below so that Customer may be identified.


  1. (1)One of the following copies of the document
    (Limited to information that contains the same name and address as the person who is requested to disclose as stated in the Application for Disclosure, etc.)
    Basic Resident Register Card, Driver's License, Passport, Health Insurance Card and Alien Registration Certificate

  2. (2)Any one of the following documents
    (Information prepared within 30 days prior to the date of the request for disclosure, etc.; all documents are copies of the originals issued by a public organization such as a municipal office.)
    A copy of the residence card, a copy of the alien registration card

(C)Fees

Please enclose in the invoice a stamp of the amount equivalent to the following fee.

  • 800 yen per invoice

4.If requested by an agent

Please attach the following document for proxy confirmation.


  1. (D)Power of attorney (seal with registered seal) Download

  2. (E)Certificate of seal impression (the same seal impression as that of the seal affixed to the power of attorney)

  3. (F)Document for identification of the agent itself (described in (B) above)

5.If requested by a legal agent

No power of attorney is required. Attach the following document to confirm the legal proxy.


  • (D') A certified copy of the family register, a certificate of the family court, a certificate of registered matters, or any other document certifying the qualification
    (limited to those prepared within 30 days prior to the date of the request for disclosure, etc.)

  • (E')Document for identification of the agent itself (described in (B) above)

6.Our response to the request

The response documents from us will be sent by registered mail with return receipt requested to the address of the requester himself or herself (or his or her agent, if the requester's representative) at the address stated in the attached document.

7.Cases Where the Request for Disclosure, etc. Is Unable to Be Responded

In the following cases, you may not be responded to the request for disclosure or notification for the purpose of use.


  • In the event that there are any deficiencies in the descriptions and attached documents

  • When the authority of agency shall not be confirmed (when requested by an agent)

  • Cases in which a request for disclosure does not fall under the category of Personal Information

  • Cases in which disclosure is likely to harm the life, body, property, or other rights or interests of the person or a third party

  • Where there is a risk that the proper execution of our business will be significantly hindered.
  • Cases that it causes violations of other laws and regulations
